- player name and parent contact info

- Team Name (12U Zondlo, 12U Pell, 12U Sword, 10U Groholski, 10U Kovarik, 8U Red, 8U Black)

- Players must choose two preferred jersey numbers in case there are duplicate requests. (Seniority applies).

12U Sword: give the site a bit to update, it was created prior to this team being added, so currently unavailable in the dropdown menu, so sorry for the confusion!!

please note, the uniform package includes two jerseys*, one jacket, and one pair of socks. This is no additional cost, it is included in your registration fee. Prism has extra gear offered on their site which is optional and is an extra cost to you.
