Jersey Information – Due back by Sunday August 2^nd.

You will need:

- player name and parent contact info

- Team Name (8U, 10U Red, 10U Black, 12U)

- Players must choose two preferred jersey numbers in case there are duplicate requests. (Seniority applies).

Please note, Fall Ball uniforms consist of one jersey per player. This is no additional cost; it is included in your registration fee. Prism has extra gear offered on their site which is optional and is an extra cost to you.
